Case Management Intern
- Assist Rehabilitation team with identifying the rehabilitation/vocational goals and measuring the outcome of the goals periodically to be in compliance with clients’ IRP
- Assist Rehabilitation team with auditing of PRP documentation
- Assist with administrative tasks such as filling out client demographics, contact, insurance, authorization, assignments, dx, and documents
- Assist with managing and creating new content for each of our social media platforms (Twitter, Facebook, Instagram)
- Assist with creating a condensed community resources binder for PRP consultants that include the nearest shelters, substance abuse facilities, group homes, day programs, assisted living centers, etc., primarily in Prince George’s, Montgomery, Baltimore, Howard, Anne Arundel, Charles, St Mary’s, and Calvert County (add additional counties as needed)
- Assist with developing and recording outcome measures for clients in each program
- Assist clients with researching community resources to best serve clients
- Assist with any other program as needed (Substance abuse, Intensive Outpatient Program IOP, Outpatient Program OP, Health Home, Supported Employment, etc)
- Assist Rehabilitation team with documenting coordination of care between other healthcare providers
