Great opportunity to join Riverside Community Care as a Health and Wellness Coordinator for our amazing Developmental and Cognitive Services Division in the South Shore area!
 
Work as part of our cohesive, experienced, multi-disciplined team in our Acquired Brain Injury Residential Programs.  We offer an extensive array of residential options from 24 hour staffed homes to semi-independent living situations located in a variety of settings, from apartments to homes. 
 
The Health and Wellness Coordinator will:
  • assess the health care needs of the people identified and coordinate with the staff and outside providers regarding the individuals’ medical needs
  • educate and train individuals and staff to increase their awareness of health-related issues to promote optimal health for all individuals
  • monitor the medication administration system to ensure the safety of the people we serve in the division

Required Skills
Excellent communication, organizational, and time-management skills required. Basic computer fluency (Microsoft Office) required. Driver's license required. Must be able to lift ten pounds.
Required Experience
Nursing Degree required. Minimum of one year experience in the mental health field required.
