SUMMARY:
Risk Manager at Hampstead Hospital and Residential Treatment Facility (HHRTF) directs risk management functions, including evaluation of patterns/trends, informing staff about risk events that occur and providing recommendations to reduce such events, and collaborating with leadership to establish a culture of safety. The role performs orientations and trainings in risk management and occurrence reporting, audits and evaluates practices for compliance with Hospital standards and procedures, and assists in the review of hospital policies and procedures.

MINIMUM QUALIFICATIONS:
Education: Master's degree from a recognized college or university with major study in health administration, business or public administration, nursing, public health or law. Each additional year of approved formal education may be substituted for one year of required work experience.
Experience: Four years' professional experience in Risk Management in a health care or mental health setting with responsibilities in program research, planning, monitoring, and evaluation.
OR
Education: Bachelor’s degree from a recognized college or university with major study in health administration, business or public administration, nursing, public health or law.
Experience: Five years’ professional experience in Risk Management in a health care or mental health setting with responsibilities in program research, planning, monitoring, and evaluation. Each additional year of approved work experience may be substituted for one year of required formal education.
License/Certification: None required.
RESPONSIBILITIES:
- Coordinates the Risk Management Program and acts as standing member of the Safety and Performance Improvement Committees. Facilitates the annual review and revision of the facility’s Risk Management Program and related policies. Updates risk management planning to help avert future problems.
- Manages occurrence reporting, including ensuring accurate and timely reporting and entry of incidents into the electronic occurrence reporting system, and timely reporting of adverse incidents per regulation. Aggregates occurrence report data to provide scheduled and ad hoc reports and metrics. Analyzes data to identify and report trends to leadership and to build solutions. Maintains organized and accessible information and supporting documentation for frequent review.
- Leads root cause analyses and post incident analyses, in collaboration with the Quality Department, including designing and implementing appropriate risk reduction strategies and identifying corrective action planning. Reviews event reports each workday in preparation for discussing these events with clinical and/or administrative leadership.
- Oversees the implementation of risk management initiatives by hospital personnel to ensure desired purpose and outcomes, and measures efficiencies. Submits timely, accurate and thorough reports to leadership and outside agencies regarding continuous improvements. Maintains up-to-date knowledge of The Joint Commission (TJC) standards and regulatory requirements to establish guidelines for reporting activities.
- Performs audits and reviews of hospital practices and procedures, identifies deficiencies, liabilities, risks and determines corrective measures; provides training, either one-on-one or in a group setting. Participates in annual Hazard Vulnerability Analysis and Failure Mode and Effects Analysis to identify facility risks and implement mitigation strategies.
- Manages investigations related to adverse events, including interviewing staff and reviewing medical records for evaluation of clinical improvement and regulatory/liability reporting. Ensures that timely investigations of incidents to patients, staff, and visitors occur, and that the facility remains risk-free of adverse publicity or serious financial liability, including allegations of served persons' rights violations, abuse, neglect, exploitation, and grievances.
- Coordinates with external agency investigations; responds to and investigates complaints and takes appropriate actions based on findings of investigation. Keeps abreast of appropriate legal, regulatory requirements related to risk management. Responds to inquiries and provides resources, consultation and support to hospital and professional staff on risk and ethical issues.
- Helps with the identification and development of information related to equipment failures to ensure compliance with the reporting requirements of the Safe Medical Devices Act.
The work of an employee in trainee status in this position shall be overseen by a fully qualified individual. An employee in trainee status shall meet the minimum qualifications within the period of time specified on the SJD, not to exceed one year of being hired into this position.
