Associate Director of Clinics & Home Care Services

Key Responsibilities:

Provide visionary leadership to clinic and home care teams, including the Home Care Services Manager, Care Coordination Manager, and Clinic Nurse Manager.

Develop and oversee clinical workflows and care delivery systems to ensure regulatory compliance and high-quality outcomes.

Drive team engagement and performance through mentorship, training, and regular feedback.

Collaborate with the Director of Operations and Medical Director to maintain a high-functioning interdisciplinary team (IDT).

Standardize and implement effective clinical processes across all centers.

Lead policy development, quality improvement efforts, and change management initiatives.

Ensure strong communication, customer service, and alignment with our core values across teams.

What You Bring to the Role:

RN license (required) and a Bachelor’s degree in healthcare or business.

Minimum 2 years of leadership experience in healthcare settings, with specific home care experience required.

Strong knowledge of clinical operations, team leadership, and regulatory standards (CMS, MDHHS).

Expertise in EMR systems, clinical documentation, Microsoft Office Suite, and payroll systems.

Demonstrated ability to lead with compassion, integrity, and strategic thinking.

Experience working with the frail elderly or long-term care populations preferred.

Additional Requirements:

Current CPR certification (or willingness to obtain through SCP PACE).

Must be fully vaccinated or willing to become vaccinated for required communicable diseases.

Reliable transportation, valid Michigan driver’s license, and current auto insurance.

Willingness to work flexible hours including weekends, holidays, and during emergency situations.

Work Environment & Physical Demands:

Fully onsite leadership role with regular travel between care centers.

Must be able to sit, stand, push/pull, and lift up to 20 pounds as needed.

Frequent interactions with staff, caregivers, and participants in clinical settings.

Resilience to manage high-level administrative responsibilities with hands-on leadership presence.
