Job Duties:

Wilson Workforce and Rehabilitation Center (WWRC) is seeking an Admissions Program Support LPN.

The Admissions Program Support LPN will provide critical support in the admissions process, focusing on case management, referral review, and consumer satisfaction. This role serves as the primary point of contact for complex medical referrals and ensures participants are prepared for services in alignment with WWRC’s mission.

Key Responsibilities:

• Review and interpret pre admission documentation to determine eligibility under WWRC Admission Criteria
• Coordinate with DARS Counselors, participants, and healthcare providers to develop individualized service plans
• Manage medical orders and forms during the pre admission process to ensure compliance with medication administration policies
• Provide orientation on medication administration policies to participants and families during intake and new student orientation
• Document communications and maintain accurate records in the AWARE Case Management System
• Support Admission Intake Days by conducting enrollment interviews, assisting families, and guiding participants through the process
• Answer the second Admissions phone line and provide information about WWRC services in an efficient, informative, and polite manner
• Respond to participant concerns promptly and respectfully, ensuring a positive consumer experience
• Contribute to consumer satisfaction initiatives, including the Voice of the Consumer program


Minimum Qualifications:

• Knowledge of medical and psychological terminology, including the effects and social aspects of disabling conditions

• Knowledge of available community resources and rehabilitation processes

• Must have demonstrated superb oral and written communication skills

• Ability to analyze and interpret written referral documentation to enable evaluation and goal setting

• Ability to apply written criteria to a decision‑making process

• Ability to exercise concise independent judgment and apply effective time management techniques

• Knowledge of insurance guidelines, including federal, state, and private funding requirements

• Skilled in word processing and data entry

• Basic knowledge of computer systems

• Broad authority to approve most admissions and schedule according to program guidelines

• Seeks guidance from professional staff for program decisions or unique scheduling situations


Education, Experience, Licensure, Certification Required for Entry into Position:

Virginia Licensure as a Practical Nurse or eligibility

 

Additional Considerations:

Prior LPN experience in a residential setting.
