Job Purpose

 

Identifies and engages the target population, assesses needs, and provides information and assistance to access available resources within the community.

 

 

Primary Responsibilities and Duties

 

  • Provides screening and assessment of persons served. 
    • Conducts interviews with individuals (and legal guardians as indicated) to collect information both initially and ongoing to address suicidal or homicidal ideations and social determinants of health.
    • Ensures all individuals referred are seen face-to-face within program guidelines.

 

  • Ensures coordination of services for persons served. 
    • Collaborates with the community and outside agencies to ensure support and services are in place.
    • Prepares and maintains documentation within program guidelines.
    • Serves as a resource to guide and support efforts to engage communities and local agencies in referrals and treatment. 

 

  • Provides clinical services within program guidelines. 
    • Provides face-to-face contact within program guidelines.
    • Educates and models healthy basic living and personal care tasks and behaviors.
    • Encourages individuals to remain in a continuum of care to ensure success.

 

Primary Attributes

 

Professional & Technical Knowledge:

 

  • Possesses the equivalent of formal training in behavioral health, usually in the form of a major in Psychology, Social Work, Counseling, or Criminal Justice as part of a Bachelor’s Degree program.

 

Continuing Education: 

  • Maintains current knowledge of standards of care and practices, typically acquired through continuing education.

 

Licenses & Certifications: 

  • A valid state-issued driver’s license is required.

 

Technical Skills:

  • Prepares basic correspondence and simple reports in Microsoft Word. Uses Microsoft Excel to create tables and simple displays of information. Creates basic presentations in Microsoft 

PowerPoint. Demonstrates necessary proficiency with all electronic clinical systems, including EHR and scheduling systems, in use at the Company.

 

General Development:

  • Displays basic organizational skills to self-prioritize and organize own work. Determines effective responses to situations encountered within established precedent. Able to effectively communicate opinions drawn from conclusions using inference and logic. Exercises tact and diplomacy in resolving mild conflicts or disagreements.

 

Work Environment:

  • Work is performed independently in the field, primarily in clients’ private homes. Physical demands include frequent walking, standing, and possibly climbing stairs in homes without elevators. The position may occasionally involve lifting or carrying materials.
