Hospice Centralized Support Specialist

Pay range (depending on experience): $28-32 per hour 

Schedule/Shift: Full-time, M-F or Weekend Rotations

This is a remote opportunity based out of San Diego, CA

As part of our centralized team, each member will be responsible for inquiring and gathering information from the EMR, Physicians, referral sources, agency staff and others involved to ensure patient records are received timely and are met in accordance with payer requirements and the Agency’s SOP.

Responsibilities:

  • Procure, organize, and maintain the documents necessary for face-to-Face clearance.
  • Review the EMR for attached documents, and review any F2F encounter notes as well as the SOC, diagnoses, and the physician plan of Care.
  • Maintain the confidentiality of patient and organization information.
  • Process electronic workflow and manage electronic documents in required timeframes.
  • Comply with applicable legal requirements, standards, policies, and procedures including, but not limited to HIPAA and documentation standards.
  • Support prebill/orders management team as needed.
  • Participate in monitoring and auditing activities and investigations, and implementing quality improvement processes, as required.
  • Report concerns and suspected incidences of non-compliance.
  • Cooperate with monitoring and audit functions and investigations.
  • Maintain excellent customer service with colleagues, referral sources, physician offices and patients.
  • Additional responsibilities as needed.

Qualifications:

  • High school graduate with minimum 5 years of progressive business experience. Bachelor’s degree preferred.
  • Proficient in work processing, desktop publishing and spreadsheet software.
  • Minimum one year experience in Home Health
  • Experience in dealing with variety of customers in healthcare, including physicians, clinicians, and healthcare facilities.
  • Knowledge of medical terminology and Medicare guidelines
  • Excellent verbal and written communication skills.
  • Knowledge of clerical and administrative procedures and systems.
  • Ability to anticipate and identify problems and use sound judgment and fact-based analysis to develop effective and efficient solutions.
  • Demonstrated ability to work effectively in a team environment while meeting task deadlines.
