Role: Anesthesiologists No of Contractors required: 5 FTE Work site: Dorn Va Healthcare Columbia 6439 Garners Ferry Rd Columbia SC 29209 Period Of Performance: Base 1(one) year + 4(four) one-year options 

Pay Rate:  $305.09 per hour

 PERFORMANCE WORK STATEMENT GENERAL:

 Board Certification: Physician(s) shall be Board Certified /Board Eligible by the American Board of Anesthesiology (ABA), and be currently certified in Basic Life Support (BLS) Advanced Cardiac Life Support (ACLS) or equivalency. All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the physician to the VA COR on an annual basis for each year of contract performance. 2.1.3. Credentialing and Privileging: Credentialing and privileging is to be done in accordance with the provisions of VHA Directive 1100.20 and VHA Directive 1100.21
