Proposed Schedule Includes:
8-hour shifts beginning between 7:00am - 8:00am
Potential for weekends (additional $2.00/hour)

Summary:
To perform complex interventions relating primarily to the psychosocial and emotional needs of Veterans who have served. Provide case management services to Veterans and their families as they transition through the continuum of care at the NH Veterans Home.
YOUR EXPERIENCE COUNTS: Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.
MINIMUM QUALIFICATIONS:
Education/Experience: Bachelor's degree and 1.5 years of experience OR equivalent combination of 5.5 years of education and experience after completion of high school. Education and experience must be in the fields of psychiatric social work (mental, emotional or behavioral disorders.
License/Certification: Valid Driver’s License

Responsibilities:
Completes mood, behavioral and psychosocial well-being sections of the MDS (Minimum Data Set) upon admission, quarterly and in the event of a significant change. Provides clinical summary and/or discharge related documentation.
Completes psychosocial assessments on residents through in-depth conversations with the resident and their families. Assist in determining the needs of the resident. Uses diagnostic information to participate in formulation of treatment goals based on identification of problem area and develops interdisciplinary care plans and coordinates weekly care plan meetings.
Provides on going education/support to residents, staff and families and community to promote/optimize Quality of Life for all residents across the continuum of care. Mobilizes resources to uphold resident’s comfort and dignity.
Provides a wide range of short-term therapies during treatment or crisis intervention. Develops, coordinates and implements monthly group sessions with residents on specific topics relative to their military experience, aging, familial relationships and other issues as deemed necessary.
Provides focused support and education regarding the psychosocial emotional needs of residents receiving Palliative Care.
Identifies need for DPOAH and/or Guardianship status and assist residents and families with the Guardianship process.
Consults with other professional staff regarding treatments, psychosocial diagnostic formulations, discharge planning if applicable, availability of community resources and types of community programs. Use professional judgment to implement a sequence of operations or actions.
Coordinates family involvement and establishes contacts with community based agencies for appropriate care and/or aftercare. Present and/or facilitate meetings to present information and goals to employees, families and residents to increase the responsiveness toward the demands of the residents.
Monitors, evaluates and provides supervision to subordinate level social workers. Analyze and develop new work methods and procedures subject to periodic review. Make decisions to solve problems as well as articulating and expressing those solutions in order to meet professional and/or treatment goals.
Performs weekend and on call social work services to dementia and non-dementia units within the facility on a rotating basis.
