CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

JOB SUMMARY
An Adolescent Navigator is an individual who assists in addressing all the ancillary concerns and needs of the adolescent by providing ongoing support, needs identification, intra-agency, external coordination in conjunction with the established treatment team. This employee is responsible for ensuring that supportive services and follow-up are provided prior to discharging a patient/participant from a CBI Behavioral Health Services. The goal of the Adolescent Navigator is to enhance the likelihood of success, identify and reduce barriers for clients receiving Medication Assisted Treatment (MAT) and BH services. The Adolescent Navigator will provide support and ongoing patient engagement while adolescent is participating in CBI Services. The Adolescent Navigator is responsible for participating in Child and Family Teams (CFT), providing administrative support to the Licensed Practical Nurse (LPN) and Nurse Practitioner (NP), coordinating referrals, inclusion of family supports in treatment planning and connection/coordination to community supports.
ESSENTIAL JOB DUTIES OR RESPONSIBILITIES
Alerts and coordinates services with any assigned treatment team identified by State Roster.
Ensures that patient identified Primary Care Physician (PCP) is notified via fax.
Assists patient with obtaining PCP if one is not assigned.
Ensures that if a patient is open with other agency that notification with that agency is made immediately and documented.
Obtains all necessary documentation from outside providers and other entities involved regarding patient history.
Coordinates with Level 1 inpatient hospitalization on individuals in need of emergent appointment.
Coordinates with the adolescent’s family of choice and includes in all aspects of treatment.
Staff with LPN and Medical Practitioner on significant patient observations.
Participates in all CFT’s, completing necessary reports, documenting what was contributed by each staff member, and relays necessary information to all parties involved in treatment.
Protects each patient’s confidentiality within the guidelines of established policy and process.
Provides input to the assigned LPN and Medical Practitioner on patient/family ancillary needs and any psychosocial stressors (housing, educational, vocational, access to health care, social, economic problems, legal system/crime, and other psychosocial and environmental problems).
Coordinates referrals for placement and connects to community resources identified in treatment plan.
Provides in-home support for assistance with the self-administration of medication, assistance in filling prescriptions, transport to lab for testing and transport to medical and psychiatric follow-up appointments based on need.
Provides physical outreach/re-engagement services for high acuity clients as identified by the attending Medical Practitioner.
Tracks and follows up on client Labs, in coordination with medical staff.
Reviews and updates Scheduler with Show/No Show status
Gathers needed information for the development of monthly progress reports.
Facilitates a phone call to remind clients about upcoming appointments.
Follow-up with MAT clients to ensure they are compliant with prescribed medications.
Facilitates a phone call with each patient within 24 - hours after any missed appointment.
Coordinates results from the follow up with the assigned clinician and documents outreach efforts accordingly. If the patient/participant is able to be contacted and indicate missing the scheduled session due to a crisis such as relapse or psychiatric issue or related psychosocial stressor, Adolescent Navigator will staff with his or her supervisor to determine the appropriate course of action in assisting the patient/participant accessing needed services.
Follows-up with referring agency after completion of initial assessment to notify them that a participant is enrolled.
Assists the assigned LPN/Medical Practitioner in coordinating treatment planning with the referring agency and any other involved stakeholders in the client’s treatment.
Re-engages patient if in need of additional services and coordinates such services with the assistance of the Community Bridges or other assigned clinical team.
Document the specific interactions with patients being detailed and thorough reporting the facts.
Correct all deficiencies identified by Medical Records personnel within five days of being notified.
Document all NextGen forms in accordance with Clinical process to minimize encounter errors.
Complete and deliver all Incident Reports or Significant Events, prior to the conclusion of the shift where the incident occurred, as specified in Community Bridges Policies and Procedures.
Complete and document all patient interaction and progress notes within 24 hours, if unable to do so Supervisor must be contacted immediately.
Adheres to established policy, process, and ethics for this assignment.
Other duties as assigned by Supervisor and/or Manager and administrative support.
Document specific Adolescent Navigator referrals and services provided.
Document weekly outcomes to Management.
Complete transport requests and enter trip tickets in NextGen.
Participates in assigned Care Team meetings and huddles.
Other duties as assigned by Supervisor and/or Manager and administrative support.
MINIMUM EDUCATION & EXPERIENCE REQUIRED
High School Diploma or GED is required. Associates Degree is preferred.
1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
Experience working with adolescents preferred.
LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS
Current AZ Driver’s License (valid and in good standing).
Clear 39-month Motor Vehicle Record.
Arizona Fingerprint Clearance card, Level 1
