Title: Case Manager - Assertive Community Treatment (CADC)
Location: Totem - 1438 SE Division, Portland, OR 97202
Schedule: This is an in person position, Monday through Friday 8:30am-5pm.
Wage Range: $24-$34, non-exempt

Position Overview:

The CADC Case Manager will provide case management and addictions counseling services, including individual and group counseling. Responsible for completing ASAM assessments, and for coordinating referrals to residential or intensive outpatient chemical dependency treatment services. Also assists clients in different stages of change in relation to substance abuse, focusing on building motivation.

 

What you will do:

  • Seek and accept clinical and administrative supervision regularly and integrate feedback into daily performance.
  • Engage in collaborative relationships with ACT team members and staff across NARA programs to work towards integrated systems services.
  • Formal and informal ongoing clinical and non-clinical assessments of clients within the appropriate scope of practice; developing, writing, implementing, and revising treatment plans with clients
  • Assist clients in obtaining income and housing
  • Reliable transportation is required.

 

Qualifications

We would like to hear from people that have:

  • Bachelor’s Degree in social work, psychology, or other social services related field
  • QMHA required (QMHP preferred)
  • CADC required
  • At least one year experience providing direct services to individuals dually diagnosed with mental health concerns and substance use issues
