Program Manager, HCP Capacity Building

PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.

New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.

This is a grant-funded position ending March 31, 2027.

Program Description: 

The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS’ experience in sexual and reproductive health and healthcare-community partnerships to the execution of scalable healthcare capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York. The HCP Capacity Building team will support the trustworthy integration of health-related social needs (HRSN) screening and referral to community resources into a broad range of healthcare practice settings within PHS’ SCN, including hospital care, primary care, sexual and reproductive healthcare, behavioral health, and care management programs.

 Position Summary: 

We seek an experience Program Manager who will be responsible for the management, administration and execution of day-to-day activities for one or more Quality Improvement Learning Collaboratives (QILCs) focused on supporting healthcare partners to implement HRSN screening using a required tool, refer to PHS’ SCN, and navigate clients to ensure identified HRSN needs are met. Under the supervision of the Director, HCP Capacity Building, they will manage project administration including project timeline; monitor programmatic and evaluation progress and deliverables; internal communication and reporting; resolve issues that may affect project timelines and milestones; assist in the planning and delivery of learning sessions, webinars, trainings; provide other logistical and administrative support as needed, ensuring coordination with all relevant PHS staff from the Legal/Contracts and Fiscal departments.


The Program Manager will report to the Director, HCP Capacity Building and coordinate with the Quality Improvement (QI) and Evaluation Manager, Practice Improvement Specialists(s), and Training Manager, as well as work in collaboration with the overall HCP and HCP Capacity Building teams as necessary.


Specifically, the Program Manager will: 

  • Manage, under the supervision of the Director, HCP Capacity Building and in collaboration with the HCP Capacity Building team, project workplan activities, internal reporting, quality, compliance, and evaluation. Communicate and coordinate with the Director to assure smooth and effective operations and interactions with external partners.
  • In coordination with Practice Improvement Specialist(s), oversee healthcare partner submissions of project deliverables, including quality improvement tools, data reports, attendance at trainings, webinars and learning sessions, and follow-up with partners as needed.
  • Manage preparation for project learning sessions and technical assistance calls in collaboration with Quality Improvement and Evaluation Manager, Practice Improvement Specialist(s) and Training Manager, including timeline management, logistics, preparation of materials, coordination with partner sites, coordination with consultants (as necessary), evaluation and other relevant tasks.
  • Support preparation for trainings and webinars in collaboration with Quality Improvement and Evaluation Manager, Practice Improvement Specialist(s) and Training Manager, including timeline management, logistics, preparation of materials, coordination with partner sites, coordination with consultants (as necessary), evaluation and other relevant tasks.
  • Independently facilitate or co-facilitate internal and external meetings or events as assigned; this may include but is not limited to portions of learning sessions, webinars, trainings.
  • Support internal reporting as necessary, including but not limited to brief reports on key project milestones, success/challenges, outcomes and performance data.
  • Act as primary point of contact for external partners regarding administrative elements of SCN capacity building initiatives.
  • Attend relevant HCP meetings, including Quarterly All-Hands, Roadmap Advocacy Coalition, and others as assigned.
  • Remain up to date on research, relevant public policy and best practices for HRSN screening and referral and service provision network in NYC.
  • Travels within the US to meetings, conferences and for project activities as needed, approximately once per year.

Qualifications and Experience: 

  • Bachelor’s degree or equivalent in Public Health or related field; Master’s degree preferred.
  • Minimum 3-5 years of experience in managing programs/projects related to healthcare and/or social services.
  • Direct experience supporting continuous QI processes with healthcare facilities preferred.
  • Foundational knowledge of community, city and state health department systems and NYC-based provider networks. Prior experience in issues related to healthcare; social services; health equity, access and quality; population health (or some subset/combination of these) preferred.
  • Salary: $77,250 - $77,250

  • Highly organized and detail-oriented with exceptional time and project management skills. Ability to work independently and as part of a team, managing multiple priorities and deadlines.
  • Ability to effectively collaborate within and across the HCP Capacity Building team, Healthcare-Community Partnerships Division, other organizational units, and with colleagues in various external working groups/forums
  • Proficiency in Microsoft Word, Excel, PowerPoint and use of web-based meeting applications.
  • Commitment to improving health equity, access and quality of care within healthcare practice settings in NYC.
