The Nursing Facility Survey Unit (NFSU) is staffed by healthcare professionals who have been trained and certified by the Centers for Medicare & Medicaid Services (CMS) to conduct federally mandated complaint investigations and certification surveys, to determine compliance with requirements for long-term care nursing facilities (NFs) participating in the Medicare and Medicaid programs and/or licensed in the State of Oregon. We are seeking multiple Nursing Facility (NF) Client Care Surveyors (Client Care surveyors) to join our Nursing Facility Survey Unit Locations:

Salem – 500 Summer St. NE Salem, OR

Tigard – 10775 SW Cascade Ave. Tigard OR

 

 

Note: Employees who are not yet a PERS participating member, will receive the 6.95% pay increase and begin the 6% employee contribution upon participating in PERS:

Client Care Surveyor is salary range 28 AA: 5,232.00 - 8,024.00

Client Care Surveyor is salary range 28 AP: 5,598.00 - 8,581.00

 

 

 

 

 

Summary of Duties

 

As a Client Care Surveyor, you will:

Participate in combined federal certification and state licensing surveys of nursing facilities.

Perform surveys conducted according to mandated federal and state requirements, protocol, guidelines, and time frames.

Coordinate surveys as the Team Coordinator by ensuring survey process guides are followed in the completion of survey events.

Facilitate communication between survey team members, other NFSU staff and facility staff.

Plan and coordinate survey workload distribution and logistics on an ongoing basis.

Gather information in survey process by reviewing facility’s survey history, complaint investigation reports, and data reports

Observe details of residents’ quality of care, life, and safety including staff assistance with activities of daily living, medical/nursing procedures, staff interactions with residents, nutrition, activities provided, and environment.

Contacts other personnel or agencies such as Adult Protective Services (APS), CMS, Long-Term Care Ombudsman (LTCO), state program coordinators, Fire Marshal, physicians, pharmacists, healthcare professionals, etc., as needed, to exchange and/or validate relevant information.

Write reports of findings using specified CMS computer software program, and according to CMS “Principles of Documentation” guidelines. Reports must be clearly descriptive of findings, concise, complete, and legally defensible.

 

 

 

 

 

 

Minimum Qualifications

A valid driver’s license and acceptable driving record are required for this position.

 

Three years of experience in areas related to nursing, social work, developmental disabilities, dietetics/nutrition, or sanitation AND one of the following:

A Bachelor's degree in a health or human services field; or

A license as a Registered Nurse.

LPN certification and/or experience will not meet the minimum qualifications.

 

Encouraged: Experience in areas related to Nursing, Social Work, Speech Pathology, Physical Therapy, Occupational Therapy, Investigations or Nursing Home administration.

 

 

 

 

 

 

 

 

 

Essential Attributes

 

We are looking for candidates with:

Experience in areas related to Nursing, Social Work, Speech Pathology, Physical Therapy, Occupational Therapy, Investigation, Long-Term Care or Nursing Home administration.

Skill in exercising independent judgment and decision-making in the application of laws, rules, regulations, policies, or procedures.

Experience writing professional reports and communicating verbally with a variety of medical and other professionals, facility management and staff, residents, relatives, and other entities, sometimes under tense or hostile conditions.

Experience and knowledge of federal and state laws, rules, and regulations related to the provision of care and services in health or health-related settings.

Display a willingness to help others, put forth the effort to help others, remain calm in tense situations, and communicate openly, positively and directly with co-workers.

Proficient using electronic media/computers for communicating such as Teams, Word, Excel, PowerPoint, Outlook, SharePoint, OneDrive, Zoom and cell phone.

Working Conditions

Hybrid remote/on-site work is required for this position. The first month you will need to be in the Tigard or Salem office to complete required training.

Reporting to the Salem or Tigard office is required when business or operational needs are dependent on in-person meetings, assignments, trainings or activities. 

You will primarily work within a variety of statewide long-term care facilities and working conditions (such as small conference rooms, standing for long periods of time, working on a laptop computer, etc.).

Lift and carry up to 20 pounds throughout each day.

Irregular work schedules and frequent schedule changes, including occasionally working long days, nights, overnight travel, and weekends/holidays.

You may interact with a wide range of people including some who have experienced trauma and exhibit challenges managing their emotions.

You will be a mandatory reporter of abuse and neglect of children, older adults, and other adults with disabilities.

Extensive daily use of electronic devices (computer, wireless communication devices, etc.).

You will be required to frequently travel and drive statewide at times in inclement weather, long distances, and overnight for variable lengths of time to and from NFs, offices, for meetings, training, and surveys.

State and rental cars are available for use as well as some mileage, accommodations and meal reimbursements are provided.

This position is not eligible for overtime. However, straight time is awarded for use in addition to other paid time off.
