Wages
$50,000 - $53,040 annual salary
Benefits
Paid vacation and sick time, plus 12 paid holidays per year; medical, dental, and vision insurance offerings; Health Savings Account (HSA); There is employer paid basic life insurance with the option for additional coverage paid by the employee; and Interact offers a retirement plan for its employees where employees can make contributions to their account.
Schedule
This position is fully in-person and may include some evening and weekend hours.
A Typical Day in the Life…
A typical day as Interact’s Designated Coordinator provides an opportunity to play a vital role in ensuring artists receive services that support their artistic growth, autonomy, and well-being. and will include the following:
- Coordinating and monitoring the delivery of 245D services as defined in each person's Coordinated Services and Support Plan (CSSP) and CSSP Addendum.
- Developing and revising person-centered support plans based on the needs, preferences, and goals of each individual.
- Conducting and documenting regular site visits and face-to-face interactions with participants to ensure service satisfaction and effectiveness.
- Overseeing daily documentation and data collection for service delivery.
- Monitoring incident reports and ensuring timely reporting and follow-up per DHS and internal policies.
- Providing supervision, coaching, and training to Direct Support Professionals (DSPs) to ensure they have the knowledge and skills to implement service plans and uphold individual rights.
- Partnering with guardians, families, case managers, and other professionals to support individuals holistically.
- Participating in annual and semi-annual team meetings, evaluations, and planning sessions.
- Supporting artists in expressing choices, resolving concerns, and advocating for themselves.
- Assisting with licensing reviews, audits, and other quality assurance activities.
- Maintaining organized, up-to-date files and records.
- Serving as a Direct Support Professional in the Visual Arts Area.
Who You Are
- To meet DHS requirements for a Designated Coordinator, you must meet at least one of the following criteria: You have a bachelor's degree in a field related to human services and one year of full-time experience providing direct care services to persons with disabilities or persons age 65 and older; You have an associate's degree in a related field and two years of full-time experience; You have a diploma in a related field and three years of full-time experience; You have a minimum of 50 hours of education and training related to human services and disabilities and four years of full-time experience providing direct support services.
- You have strong knowledge of person-centered practices, 245D regulations, and HCBS service standards.
- You have the ability to write clear, professional documentation and reports.
- You demonstrate excellent interpersonal and problem-solving skills.
- You possess strong organizational skills and attention to detail.
- You demonstrate a commitment to the mission and values of Interact.
- You have previous experience working with adults with disabilities in an arts, education, or a community setting.
- You hold a valid driver’s license, have access to reliable transportation, and can pass a Motor Vehicle Record (MVR) background check. Please note that if your license was not issued in the state of Minnesota, you must apply for your Minnesota driver's license or permit within 60 days of becoming a Minnesota resident (not applicable for new hires living out of state).
- Applicants must comply with and pass a Minnesota Department of Human Services background check (Rule 11).
