SUMMARY:

Client provides holistic health care to a diverse population of patients in a primary care clinic setting. Within the individual specialty,  responsible and accountable for managing a broad range of personal health services. This includes: assessing and diagnosing problems; developing a plan; providing treatment, education, consultation and referral; evaluating efficiency of treatment; and serving as an advocate for the patient. This position will provide leadership to and work as a part of a Patient Centered Care Team. Limited to providing service to clients for which he/she is educationally and experientially prepared and for which competency has been maintained and/or authorized by the supervisory physician.

EXAMPLES OF PRINCIPAL DUTIES:

  • Basic skills, experience, training and education will examine, treat and advise patients on medical and behavioral health issues/concerns. 
  • Make referrals to appropriate social or medical agencies or other resources within the County health system. 
  • Establish therapeutic relationships with a diverse population of patients, including low-income population groups and special-needs groups such as sexually abused children, incarcerated persons. 
  • Prescribe medications and treatments for patients as allowed by scope of practice, pharmacy formulary, and department policy. Initiate, terminate, or continue medication or treatments. Within the specialty, manage health problems encountered by patients and retain accountability for outcomes. This process includes:- Assessment of health problems, both subjective and objective - Diagnosis- Development of a plan in collaboration with the patient- Intervention- Evaluation of the efficacy of the intervention Maintain appropriate documentation and health care records.
  • Provide on-call coverage for patients as assigned by Medical Director. Participate in staff and professional conferences, training, and peer reviews.

 

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of principles and practices involved in primary care in an ambulatory setting.
  • Knowledge of methods and techniques of diagnosing and treating a wide variety of diseases and illnesses.
  • Knowledge of preventative health care practices and guidance. 
  • Knowledge of professional record-keeping and reporting requirements. 
  • Knowledge of public health laws and regulations. 
  • Skilled in the use of clinical and laboratory equipment and tools associated with the specialty area of practice. 
  • Able to work with professional independence and use initiative and judgment in providing patient care in accordance with state statutes for Nurse Practitioners. 
  • Able to establish and maintain therapeutic relationships with patients. Able to interpret and apply laws, rules and regulations pertinent to the public health and primary care settings. 
  • Able to maintain effective work relationships with other staff members. 

QUALIFICATIONS FOR FNP:

  • Must possess a valid Oregon Registered Nurse license. 
  • Must have a Master’s degree in Nursing.
  • Must have graduated from an accredited Nurse Practitioner program. 
  • Must have a Drug Enforcement Administration (DEA) license to prescribe medications. 
  • Must have a valid driver’s license and acceptable driving record

QUALIFICATIONS FOR PHYSICIAN ASSISTANT:

  • Must possess a valid Oregon Board of Medical Examiners License.
  • Must have current specialty board certification.
  • Must have graduated from an accredited school of medicine with completion of accredited residency program.
  • Must pass background, criminal and NPDB check.
  • Must meet all requirements of OAR 847.050.0041 to obtain a Drug Enforcement Administration
